Remote Patient Monitoring

We make RPM seamless for care providers and patients. iTeleCare does all of the heavy lifting in assembling and testing patient device kits, providing device tracking and monitoring tools, providing program adherence materials, and supporting patients. You can also use your own tablets, or lease one of ours for program management. RPM allows clinicians the ability to deliver high quality care to a larger number of patients regardless of location and tap into additional revenue sources through Medicare, Medicaid and Private Insurers

Patients who participate in an RPM program benefit from proactive monitoring or oversight of their health, including vitals and general well-being. Providers may recommend the program to help prevent or reduce hospitalizations and adverse health events, provide you better access to care and strengthen the patients relationship with their doctor, all while providing additional care management and support.

Chronic Care Management

iTeleCare’s CCM platform has all of the tools you need to screen for eligibility, obtain program consent, generate care plans, log care minutes, and complete billing. The CCM program can be coordinated by your own medical office staff or use us to provide the clinical call center to manage the program.

Here are the answers to some of the most common questions we hear from our appreciated customers.

A. Telehealth refers to the use of telecommunication technologies and electronic information to support and expand access to delivery of health services. The definition of telehealth differs across federal and state agencies and continues to evolve. Telehealth can especially benefit patients and providers in managing chronic conditions, such as congestive heart failure, hypertension, diabetes, and chronic obstructive pulmonary disease. Telehealth is generally divided into four key areas:

1. Live video is synchronous, two-way interaction between a patient and provider that uses audiovisual telecommunications technology.

2. Store-and-forward is the asynchronous transmission of recorded health information, typically videos or images, through a secure electronic communication system for evaluation by another provider. This modality is most widely used in dermatology, radiology, pathology and ophthalmology

3. Remote patient monitoring is the collection and transmission of individual medical data from a patient in one location to a provider in a different location via electronic communications technologies.

4. Mobile health is health care and information supported by mobile communication devices such as cell phones.

Complex chronic care management services include the same criteria as the chronic care management service, plus an additional requirement of the establishment or substantial revision of a comprehensive care plan, medical decision-making of moderate to high complexity, and at least 60 minutes of clinical staff time.
Beginning January 1, 2011, the Affordable Care Act made available an Annual Wellness Visit (AWV) providing Personalized Prevention Plan Services (PPPS). The AWV includes a Health Risk Assessment (HRA). These are useful tools in targeting areas providers would never address.
The actual device does not need to be provided or installed in the office to qualify for reimbursement under 99453. The provider can arrange for a third-party provisioner to send the device to the patient. However, code 99453 requires “initial set-up and patient education on the use of equipment.” So simply providing written information about the existence of a service or device would not qualify for reimbursement for that code.
